I have a Sky-Tec starter on a Lycoming IO360. It has always started the engine BUT lately when I push the START button there is a pause. Sometimes it is a long pause then it kicks in. Sometimes I release the button and try again. I have a fear it will not start one time when I am a long way from maintenance or from home. Is this a symptom of something worse to come? Should I be concerned?

You have a right to be A FEARED!  The problem is NOT the starter...  Well, it maybe but I have seen and heard of the same problem many a time - AND the problem being the STARTER RELAY.  Sky-tech has had quite a few failures and they offer a NEW REPLACEMENT Starter Relay - IF!  You are using their relay.  
NOW!  If you are not using their relay you have to do some very simple checking:
1 - Remove ALL the Nuts, Lock Washers and Cables from the entire Starter System.  This includes the BATTERY!
2 - CLEAN the Posts, Nuts and Cables - Use a wire brush.  If you have/see corrosion wash down the corrosion with VINEGAR <-- Yes, Vinegar. Follow that up with Baking Soda and Water...  Lots of water. 
3 - REPLACE the Lock Washers with NEW Internal Star Lock Washers.
4 - Another TRICK...  Take a VOLTAGE reading ACROSS the Master Relay and the Starter Relay when there is Power Applied. NOTE:  Remove the cable from the STARTER Relay to the Starter.  You don't want the engine to start!!!   You should get ZERO (0) Volts.  ANY voltage indicates Resistance within the relay, that means it is time to replace the relay(s).  YOU DID do ALL of the above steps before you did Step 4, didn't you?

Personally David, I would check the battery also. It could show 12.2+ volts and yet have not enough amperage available to turn the engine.
And yes, you should be concerned, much concerned.
